Sick/Fit Note for less than 7 days

You do not require a doctor’s sickness certificate for any illness lasting less than seven days. Your employer may require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Sick/Fit Note for more than 7 days

If you are sick for more than seven days, your employer may ask for a ‘Statement of Fitness for Work’. To request a new/continuation or extension of an existing sick note please contact the practice on 01408 633444. the Doctor may want you to make an appointment as part of the assessment.

Please note that a ‘Statement for Fitness for Work’ cannot be post-dated, are not considered urgent on-the-day work for the doctors, but we will aim to process any requests within a 48 hour period.

Agreement to issue the sick note requested is at the complete discretion of the Doctor.
